Administrative Information & Procedure Checklist
       Template of an internal form used to assure reporting accuracy
Attendence Fax Form
        Template used to provide information to referral sources
Auditor Report
        Filled out by instructors in training
Calendar
        Internal form, used to record each participant's attendance
Contact Sheet
        Assures recording of all contacts referencing each participant
Dismissal Letter
        Sent to participant who may not be aware that he has been dismissed from the program        
Fee Card
        Made of card-stock
Last Session Letter
        Handed to participant at end of last session of their order.
Participant Pamphlet
        Handed to participant during registration/orientation
Referral Form (Court)
        Document that provides information needed on each referral and importantly, lets referred person know the deadline for making their registration/orientation appointment.
Referral Form (Non Court)
        Similar to above and disseminated to Probation, Department of Social Service, Parole (Community Corrections), and other agents of the court system.
Registration - Orientation Form
        Actual form used to register/orient referrals.
Registration-Orientation Appointment Slip
        Checklist that is filled out by staff that answers phone to set up appointment.
Roster
        Permanent record of each participant's attendance at sessions
Status Report
        Template used to keep referral sources informed of each person's status in the program.
